The biggest name concern I have is the household name thing...but this isn't really the place to discuss that.

Like...when you're in Create a Family, they all have to have one last name, even if they're not related.

Middle Names... maybe maybe not... maybe you should leave that up to the wonderful modders at Mod the Sims 2.... or MTS3. Dang, I can't wait!
