I think "linked" neighbourhoods would be a great way to go. Roads out of one neighbourhood would be the roads into the next one. A bit like suburbs really. With the right layout you could do partial data loads so you could seamlessly travel between suburbs. Think of how awesome something along the lines of GTA3 would be with fully functioning buildings and interactions with people you pass on the street would be!! A merging of GTA3 and the Sims (minus the bad language and car theft). Makes you quiver just to think about that one!!
